We will support and empower you to be able to connect with informal, community and funded supports enabling you to get the most out of your NDIS plan and achieve your goals.
We will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
We will support you through your recovery journey, ensuring you maintain supports during times of mental health crisis. We will support you to navigate the Mental Health system, whilst connecting you to informal and formal supports to meet your NDIS goals.
We provide a mobile social work service to meet your and your family's needs. A social worker adopts a strength-based approach to identify and enhance your assets, resources and resiliencies rather than focusing on deficits and deficiencies. Our social worker is an advocate for inclusion and equal participation in different aspects of community living.
Our Mental Health Support Workers are qualified and/or experienced and can provide you with day-to-day support.
By offering a safe and supportive environment, mental health support workers can help you gain the confidence and skills you need to manage your mental health and achieve your goals.
Short term accommodation, including respite, is funding for support and accommodation for a short time away from your usual home. It covers the cost of your care in another place for up to 14 days at a time. You might have a short stay with other people, or by yourself. It’s often funded when your usual carers aren’t available, or for you to try new things.
